Filling out this form is the first step in our intake process. It helps us determine a timeline for service based on your location and availability. Please include current contact information, your child’s insurance details, and any special considerations that may apply.
Benefits Verification
We use your intake form to verify insurance eligibility and coverage. Autism Care West partners with major carriers including Anthem Blue Cross Blue Shield, Aetna, Sierra Health & Life, UHC, UMR, and HealthScope. We do not currently accept Medicaid or ATAP funding.
Center Tour & Meet the Clinic Director
After completing the first steps, we invite you to tour our center and meet with one of our Directors. This visit helps answer any questions you have and ensures you’re comfortable with our program model before moving forward.
